Please fill this form out ONLY if you plan on signing your deposit over to another customer. Both parties must sign the document and the new customer must supply EPWSA with a "Service App" (above) and a copy of their picture ID.

Fill out & return with voided check.  Drafted on 12th of the month.  NO FEE FOR THIS SERVICE!

CC Authorization Form

Card will be automatically be run on the 12th of every month.  Beginning January 1, 2011 there will be a $1 fee for this service.
